Community Aged Care Packages
The aim of the Community Aged Care Packages program is to assist people eligible for hostel care to maintain their independent lifestyle at home. A support package is developed to suit each person’s requirements. Services include home support tailored to meet the individual needs of people eligible for a low care residential facility who wish to remain at home. The package may include personal care, medication management, meal preparation, assistance with shopping, housework, socialising, gardening and laundry.
Community Aged Care Packages support is available in the Shires of Mundaring and Kalamunda and the City of Swan. To be eligible for services, a person must be assessed by the Aged Care Assessment Team. Please call Swan District Hospital on 9347 5419 to arrange an assessment.
