Community Living Program

The program aims to support people with a disability to continue to live in the community of their choice whilst maintaining their independence and natural community links.

The program provides Accommodation Support, Intensive Family Support and management of Individual Options to people with physical support needs and/or an intellectual disability who have medium to high support needs, and who meet the following criteria:

  • Willing to live in the Eastern Metropolitan region.
  • Has sufficient funding to guarantee their needs can be met.

HCSG must be able to meet its Duty of Care to the person with a disability, other consumers and staff.

Accomodation Support

The program provides 24-hour accommodation support to people with an intellectual disability in a group home setting. The houses are located in Mundaring, Mt Helena and Midland.
Each person has an individual plan, which is developed with the consumer, family, staff and other stakeholders. The plan looks at all areas of a person's life including health, recreation in and out of home, home-life, communication and contact with family and friends.
HCSG will support families to be involved in their family member's life as much as they wish.

Intensive Family Support

This service is tailored to an individual family's needs and the support can be provided in or out of home. Examples of support provided are:

  • Taking a child with an intellectual disability out on a recreation outing for 4 hours on a Saturday.
  • Providing overnight support twice a week to a child with a disability so the parents can have undisrupted nights sleep.
  • Providing in home support to a woman with a degenerative physical disease to help with showering, meal preparation and taking on recreation outings such as swimming.

Individual Options

This service supports people who live in the community who require occasional support to help with specific tasks such as banking, shopping and medication.

If you or your family member requires support to live in the Community please contact the Community Living Manager on 9295 6155. This program assists people to apply for funding through the Disability Services Commission to implement a service in negotiation with the individual or family.
