Rainbow Program

Description:

Low cost Housing, Community Support and Carer Respite for adults with a severe and enduring mental illness and their carers.

Long term and secure accommodation to eligible persons in the Swan Health Region and in Northam. Individual support towards independent living and Carer Respite in the Swan Health Region.

Support includes social and living skills development, recreational and vocational support and assistance with accessing community facilities and resources. Carer Respite includes support for unpaid carers of individuals with a severe and enduring mental illness to relieve them of their caring role responsibilities for short periods of time.

Eligible persons can be referred through the public mental health service, a private psychiatrist, a General Practioner or other health professionals. Ongoing clinical support must be guaranteed.

Rainbow Housing:

Swan Adult Mental Health Centre, HCSG and Homeswest have combined to provide low cost, long term accommodation in the Northam and Swan regions for people with a severe and persistent psychiatric disability, aged between 18 and 65.

Health Department of WA funding has enabled HCSG to offer services which aim to provide security in housing for eligible persons in the Swan Region and individual support towards independent living. Swan Adult Mental Health Centre provides clinical support whilst HCSG acts in a supportive landlord capacity.

Eligibility:

An applicant for Rainbow Housing must have a primary diagnosis of an enduring mental disorder and require permanent accommodation linked with individual support.

Rainbow Support:

Rainbow Support includes social and living skills development, recreational and vocational support, and assistance with accessing community resources and facilities. Prioirity for support is given to those within the Rainbow Housing program. However, support may be provided to individuals with an enduring major psychiatric disability in other forms of independent living.

Referrals:

Eligible persons will be referred through the Swan Adult Mental Health Centre. Referrals for both Rainbow Housing and Rainbow Support should be forwarded to the Accommodation and Community Resource Manager at Swan Adult Mental Health Centre. [Referral Form]
