Intensive Support Service
Outreach support, case management and advocacy for people 15-21 years old who are homeless or at risk of homelessness and experiencing complex issues including substance abuse and mental and physical health problems.
Referral Criteria
The target group is young people (including young parents) aged between15-21 years who are living in, or using supports, in the Midland and hills region and who are experiencing a number of complex issues which they would like to address.
Participation in this service is voluntary and HCSG expects referring agencies to provide a commitment to involve the young person in the referral process and to ensure that they have given their consent to the referral.
HCSG requires referring agencies to discuss referrals with an HCSG case worker before they are submitted.
There is no charge for participation in this program. Please note that this is not a crisis, supervisory or behaviour management service.
How the Service Works
Staff work intensively with the young person to motivate change, gradually reducing support as the young person begins to achieve his or her goals. The worker may provide emotional and social support, practical assistance in accessing community services, referrals to other services and assistance for the young person to develop the ability to advocate for themselves. If necessary, the worker will advocate on the young person’s behalf.
