Youth Connections

Youth Connections provides case management and support for young people who are at risk of becoming disengaged, or who have disengaged, from education.

The Youth Connections’ program has been developed to meet the needs of young people at risk of not completing secondary school and not making a successful transition to higher education, training or employment.

The program provides a range of services, from individual or group counselling through to family support and intensive case management. The services form a continuum – from young people who are still attending school but who are assessed as being at extreme risk of disengaging – to young people who are totally disengaged from education. The HCSG Youth Connections’ staff work with high schools in the Swan Education District to develop strategies designed to meet the needs of ‘at risk’ students, to strengthen services available to them and to ensure that providers of other services in the region are connected.
